The following information will assist you on your first visit with us. 

Medical and Insurance Forms

At your initial visit to MD COTE we will request that you complete an intake form providing us with essential information about you and your needs before you arrive. Please call our office and we can send you the patient intake forms to have filled out before you arrive.

Arrival Time

On your first visit please arrive at least 10 minutes prior to your appointment allowing yourself time to check in.

Laser & Spa Etiquette

MD COTE Medical Laser & Spa is a place in Maple Valley for relaxation and rejuvenation. To maintain this environment we ask that all of our guests remain respectful of others privacy. Speak softly and silence all communication devices, thank you. 

Treatment Selection

Our full menu selection is listed on our web site. If you need assistance in choosing a service please call us directly our Aestheticians will be happy to answer your questions and treatments that best suit you and your goals. Rather that be relaxation, therapeutic, or a combination.    

Special Health Considerations

Please be sure to notify our staff when booking your treatment of any health concerns or pregnancy.

Payment

We accept Cash, Visa, MasterCard and Discover.                                         We also offer no interest financing for qualifying guests who wish to purchase one of our many wonderful packages.

Gratuities

Our staff is dedicated to providing you with excellent service. Gratuities are always appreciated but never mandatory. They can be given directly to the staff member, or extended at the guest services desk.

Late Arrival Time  

If a late arrival is unavoidable, please call to notify our office when possible. Arriving late will simply limit the time for your treatment as we must complete your personal service by the designated end time in order to appropriately accommodate the following guest. 

Laser & Spa Cancellation Policy

Your appointment time is reserved especially for you. Should you need to cancel or reschedule we kindly ask for 24 hrs advance notice in order to avoid a service charge.  Faliure to show without notice or same day cancellations - $50 service fee will be charged.
